What Is a IMSS Pension Beneficiary Designation Mexico (Designación de Beneficiarios Pensión IMSS)?

An IMSS Pension Beneficiary Designation Mexico (Designación de Beneficiarios Pensión IMSS) is the formal registration of dependent family members (beneficiarios) with the Instituto Mexicano del Seguro Social (IMSS) who are entitled to receive survivor pension benefits (prestaciones de sobrevivencia) upon the death of a registered worker (asegurado) or pensioner (pensionado). The Instituto Mexicano del Seguro Social was established by the Ley del Seguro Social (LSS) of 1943 — substantially reformed in 1973 and again in 1997 — as the primary social security institution covering private sector workers in Mexico. As of 2026, IMSS covers approximately 22 million active workers and their families.

Under LSS Articles 64 through 67, the IMSS survivor benefit framework recognises three categories of pension: pensión de viudez (widow/widower pension) payable to the surviving cónyuge or concubino/a; pensión de orfandad (orphan's pension) payable to surviving minor children; and pensión de ascendientes (ascendants' pension) payable to dependent parents when there are no widow or children beneficiaries. These pensions are calculated as percentages of the worker's base pension or, for pre-1997 scheme workers, as percentages of the Salario Base de Cotización (SBC) under the LSS 1973 transitory provisions.

The beneficiary designation for IMSS pension purposes is distinct from the AFORE account beneficiary designation under Ley del SAR Articles 54–56, though they are complementary documents. The IMSS designation registers family members in the IMSS sistema and establishes their eligibility for survivor pensions and healthcare benefits (servicio médico familiar) under LSS Article 84. The AFORE designation, in contrast, governs the distribution of the individual account balance as a lump sum or as part of the pension calculation.

For workers who entered the workforce after July 1, 1997 (régimen de 1997), pension benefits are calculated based on the balance of the cuenta individual administered by the chosen AFORE — sufficient contributions (1,250 semanas cotizadas under LSS Article 154, reduced to 750 under the 2020 pension reform) must have been made to qualify for a pension. For workers under the transitional régimen de 1973 — those who had already accumulated IMSS contributions before 1997 — survivor benefits may be calculated under the more favourable transitional rules, requiring only 150 weeks of contributions.

IMSS maintains the Registro de Beneficiarios (beneficiary registry) through its IMSS Digital platform, Subdelegaciones de Afiliación y Vigencia, and IMSS Bienestar modules. Workers must proactively register their beneficiaries and keep registrations updated — IMSS does not automatically update beneficiary status based on civil registry events (marriages, births, divorces) unless the worker files an actualización de beneficiarios (beneficiary update) with supporting documentation.

IMSS healthcare coverage for registered beneficiaries is provided through the network of Unidades de Medicina Familiar (UMF) and Hospitales de Zona assigned by IMSS based on the worker's registered employer address. Beneficiary registration activates healthcare entitlements — a spouse or child not registered in the IMSS Padrón de Beneficiarios cannot access IMSS medical services even if they are clearly the worker's legal dependent. The IMSS Digital platform (imss.gob.mx) allows workers to verify beneficiary registration status online using their CURP and NSS, and to initiate updates before attending an in-person appointment at the Subdelegación de Afiliación y Vigencia. Proactive maintenance of the beneficiary registry is one of the highest-impact, lowest-cost actions a worker can take to protect their family's access to healthcare and financial security.

When Do You Need a IMSS Pension Beneficiary Designation Mexico (Designación de Beneficiarios Pensión IMSS)?

An IMSS Pension Beneficiary Designation is needed by every formal sector worker covered by IMSS — ideally from the beginning of employment — and must be updated whenever family circumstances change that affect pension eligibility.

The designation is immediately needed when a worker begins IMSS-registered employment. Under LSS Article 12, all workers in a subordinate employment relationship (relación laboral subordinada) are subject to mandatory IMSS registration by the employer within 5 business days of the employment start date. Upon registration, the worker should file the beneficiary designation to ensure their spouse, children, and dependent parents are registered for both healthcare benefits and survivor pension eligibility.

The designation requires filing or updating upon marriage (matrimonio) — the spouse (cónyuge) acquires survivor pension rights under LSS Article 64 upon legal marriage registration in the Registro Civil, but must be specifically registered with IMSS through the Formato de Modificación de Beneficiarios to activate healthcare eligibility and pension entitlement records. The Acta de Matrimonio (marriage certificate) from the Registro Civil is required for registration.

Upon the birth of children (nacimiento de hijos), each child must be registered with IMSS to activate their healthcare benefits (seguro médico familiar) and establish their pension orphan rights (orfandad). Under LSS Article 84, children under 16 years (or under 25 if enrolled in school and financially dependent, or any age if permanently incapacitated) qualify for IMSS healthcare and survivor pension benefits. The Acta de Nacimiento (birth certificate) is required for each child's registration.

The designation must be updated upon divorce (divorcio) — a divorced spouse who remains registered as IMSS beneficiary may claim survivor pension benefits if not promptly removed from the registry. Under LSS Article 65, the concubina or concubino (common-law partner) acquires survivor pension rights equivalent to a spouse after 2 years of continuous cohabitation — or immediately if they have children together with the worker — and must be registered with supporting documentation.

For workers approaching retirement age or with serious health conditions, ensuring the beneficiary designation is current significantly reduces the administrative burden on family members at the time of death — enabling faster access to survivor pensions and continued healthcare coverage through IMSS.

What to Include in Your IMSS Pension Beneficiary Designation Mexico (Designación de Beneficiarios Pensión IMSS)

A valid IMSS Pension Beneficiary Designation under LSS Articles 64–67 and IMSS administrative procedures must contain the following elements to be accepted and effective in the IMSS registry.

Worker Identification (Datos del Asegurado): Full legal name, NSS (Número de Seguridad Social — the 11-digit IMSS registration number), CURP, RFC, date of birth, official identity document number (INE/IFE or passport), employer's RFC and establishment name (nombre del patrón), and current work address. The NSS is the primary IMSS system identifier and must match IMSS records exactly — discrepancies require correction through IMSS Subdelegaciones with supporting civil registry documents.

Spouse or Concubino/a Registration (Cónyuge o Concubino/a): Full legal name, CURP, date of birth, official identity document, and marriage certificate folio (for spouses) or declaration of concubinato with evidence of 2+ years cohabitation (for concubinos/as). Under LSS Article 64, the pensión de viudez equals 90% of the worker's disability pension (pensión de invalidez) or, if the worker had not yet qualified for pension, 90% of the pension that would have been payable calculated on accumulated contributions.

Children Registration (Hijos): Full legal name, CURP, date of birth, and birth certificate folio for each child. For children between 16 and 25 years enrolled in school (estudiantes), proof of enrollment (constancia de estudios) from an IMSS-approved educational institution must be updated annually. For permanently incapacitated children of any age, medical certification from IMSS's Junta de Evaluación de Discapacidad is required. Under LSS Article 65, the pensión de orfandad equals 20% of the worker's pension per orphaned child, with a combined cap of 100% of the total pension.

Dependent Parents Registration (Ascendientes Dependientes): For cases where there is no surviving spouse, concubino/a, or children, dependent parents (padre y madre) who relied economically on the worker qualify for pensión de ascendientes. Full legal name, CURP, date of birth, and birth certificate of the worker (to establish parentage) are required. Under LSS Article 66, each dependent parent receives 20% of the worker's pension, up to 40% total for both parents.

Healthcare Beneficiary Coverage (Seguro de Enfermedades y Maternidad): The same beneficiary designation activates healthcare coverage (servicio médico familiar) for the registered family members under LSS Article 84. IMSS assigns the registered beneficiaries to the Unidad de Medicina Familiar (UMF) corresponding to the worker's employer's location. The beneficiary receives the IMSS Cartilla Nacional de Salud and is eligible for all IMSS medical services including specialist referrals, hospitalisation, surgery, and pharmacy coverage under the Cuadro Básico de Medicamentos del Sector Salud.

Supporting Documents Required: Certified copies of: Acta de Matrimonio or concubinato declaration; Actas de Nacimiento of all children; the worker's current official identification; the IMSS Formato de Aviso de Modificación de Beneficiarios (Form AFIL-04 or equivalent digital form). All documents must be in original or certified copy form — IMSS requires physical presence at a Subdelegación for initial registration or significant updates.
