What Is a EPS Beneficiary Inclusion Request Colombia?

The EPS Beneficiary Inclusion Request Colombia (Solicitud de Inclusión de Beneficiario en EPS) is the formal written application that an EPS cotizante (contributing affiliate) submits to their Entidad Promotora de Salud (EPS) to add a family member as a beneficiario (health insurance dependent) under the Régimen Contributivo of Colombia's Sistema General de Seguridad Social en Salud (SGSSS). The SGSSS was established by Ley 100 de 1993 and reinforced as a fundamental right by Ley 1751 de 2015 (Ley Estatutaria de Salud), which elevated the right to health to an autonomous fundamental right in Colombia.

Article 163 of Ley 100 de 1993 defines the beneficiaries who may be included in a cotizante's EPS coverage at no additional premium cost: (1) the cónyuge (legally married spouse) or compañero permanente (permanent companion in a de facto union registered under Ley 54 de 1990 or proven by 2+ years of cohabitation); (2) hijos (children) up to age 18, or up to age 25 if studying full-time (in continuous education); (3) hijos with permanent disabilities (discapacidad permanente) at any age; and (4) dependent parents (padres dependientes) who have no income source and no independent EPS coverage. Article 164 of Ley 100 establishes that the beneficiary coverage must include the full Plan de Beneficios en Salud (PBS) — formerly the Plan Obligatorio de Salud (POS) — identical to that received by the cotizante under the Ley 1751 de 2015 framework.

Decreto 780 de 2016 (the consolidated health sector regulatory decree) establishes the procedures for EPS beneficiary registration and modifications. The PILA (Planilla Integrada de Liquidación de Aportes) system serves as the primary registration mechanism for dependent updates — employers must update beneficiary information monthly through PILA for dependent workers. For independent workers and for EPS changes initiated by the worker (not the employer), a direct request to the EPS is required using this application form.

When Do You Need a EPS Beneficiary Inclusion Request Colombia?

Marriage or Civil Union: Within 30 days of a civil marriage (matrimonio civil) registered at a Notaría, a Catholic marriage (matrimonio católico) registered at the Registraduría Nacional, or the establishment of a permanent companion status (unión marital de hecho) recognized by a notarial declaration or court order, the cotizante must request inclusion of their new spouse or compañero permanente as an EPS beneficiary.

Birth or Adoption of a Child: Upon the birth of a child (within 30 days of the birth, evidenced by the registro civil de nacimiento issued by the Registraduría Nacional del Estado Civil), or upon finalization of an adoption (sentencia de adopción), the cotizante must include the child as an EPS beneficiary to confirm continuous healthcare coverage from birth or adoption date.

Child Reaching Age 18 (Extension to Age 25): When a child beneficiary turns 18 and is enrolled in higher education (educación superior) — including university (universidad), tecnológica, or técnica institutions recognized by the Ministerio de Educación Nacional — the cotizante must submit updated enrollment documentation to extend coverage to age 25 under Article 163 of Ley 100.

Parent Dependency: When a parent (mother or father) loses their own EPS coverage — for example, upon retirement without pension, loss of employment, or death of the other parent — and has no independent income, the cotizante may request inclusion as a dependent parent under Article 163 of Ley 100.

Document Update After EPS Transfer: When the cotizante transfers from one EPS to another (traslado de EPS), all beneficiary information must be re-registered with the new EPS using this application, as beneficiary records do not automatically transfer between EPS entities.

Proof of Dependency (for parents): Sworn declaration (declaración juramentada) that the parent has no regular income source, no pension, and no active EPS coverage in either the Régimen Contributivo or Régimen Subsidiado — plus any supporting documentation (certificate of no pension from Colpensiones or AFP; certificate of no employment).

Enrollment Certificate for Adult Children: For children aged 18–25 being maintained as beneficiaries, the current semester's enrollment certificate (certificado de matrícula) from the recognized educational institution, showing continuous full-time enrollment, must be updated each semester.

Request for Effective Date: The cotizante should specify the requested effective date for beneficiary coverage — typically the same day the qualifying event occurred (birth date, marriage date, etc.) to avoid gaps in coverage, as Ley 1751 de 2015 establishes the right to continuous health coverage without interruption.
