A Life Insurance Claim Form Mexico (Reclamación de Seguro de Vida) is the formal written instrument by which a designated beneficiary (beneficiario) or legal heir (heredero) submits a request to the insurer (aseguradora) for payment of the contractual death benefit (suma asegurada por fallecimiento) following the death of the insured person (asegurado).
