2. HAZARD IDENTIFICATION AND RISK CONTROLS

2.1 The following major hazards have been identified for this project: [Major Hazards].

2.2 Work at Height: All work above 2 metres shall be performed using certified scaffolding (erected under a competent person per BOCW Central Rules 1998 Rules 57–78) or fall arrest systems. Safety harnesses shall be worn and anchored at all times.

2.3 Excavation: Excavations deeper than 1.5 metres in unstable soil shall be shored or stepped per Rules 89–102 of the BOCW Central Rules 1998. No worker shall enter an unshored trench.

2.4 Lifting Operations: All cranes and lifting equipment shall have valid third-party load test certificates. A Lifting Plan shall be prepared for all critical lifts.

2.5 Electrical Safety: A qualified electrical supervisor shall supervise all temporary electrical installations. Lockout/Tagout (LOTO) procedures shall be followed for all electrical maintenance work.

3. PERSONAL PROTECTIVE EQUIPMENT (PPE)

3.1 The following PPE shall be provided free of cost to all workers as required by Rules 41–53 of the BOCW Central Rules 1998: safety helmets (IS:2925), safety shoes (IS:1989), high-visibility vests, safety harnesses for work at height, gloves, safety goggles, and ear protection in high-noise zones.

3.2 Workers shall wear PPE at all times on site. Non-compliance shall result in removal from site.

4. EMERGENCY RESPONSE

4.1 In the event of an emergency, the site siren shall be sounded and all workers shall proceed to the designated assembly point.

4.2 The Safety Officer shall call the emergency services (Ambulance: 108 / Police: 100 / Fire: 101) and the nearest hospital: [Emergency Hospital].

4.3 All fatal accidents and serious injuries shall be reported to the BOCW Inspector within 7 days as required by the BOCW Act 1996, and to the Commissioner for Employees' Compensation within 7 days under Section 10B of the Employees' Compensation Act 1923.

4.4 All incidents shall be investigated and an Incident Investigation Report (IIR) prepared within 48 hours.

5. WELFARE FACILITIES
